Venous Thromboembolism (VTE) Prevention Program
The primary goal of the HMC Venous Thromboembolism Prevention Project is to risk stratify all HMC patients in scope for VTE and to improve thromboprophylaxis where appropriate. A year after implementation, the VTE protocol was found to be in the medical record in 98% of patients across all HMC units. Risk assessment was accurately completed in 96 percent of patients and appropriate prophylaxis was ordered in 94 percent of patients where indicated.
